The person who has received a license to distribute electricity shall submit an application to the commission in the format as per the schedule for determining the electricity tariff and other charges to be collected from the customer:
(a) The bases on which the proposed electricity tariff should be fixed (a report including economic and technical analysis and other related documents confirming the justification of those bases),
(b) If there is a need to change the electricity tariff rate, its grounds (a report with economic and technical analysis confirming the justification of those grounds and other related documents),
(c) A clear list of electricity tariffs charged in the past, proposed or changed electricity tariffs (electricity tariffs proposed to be imposed on customers using electricity for domestic, commercial, industrial and other purposes and percentage of proposed changes),
(d) Other related matters such as financial covenants agreed by the licensee with the financial institutions.
